Cortisol is a hormone released by the adrenal glands that follows a strong natural daily rhythm, highest shortly after waking and gradually declining through the day, central to the body's stress response and several other regulatory functions. This natural rhythm is exactly why a single cortisol measurement, the basis of many popular consumer "stress test" products, is a poor way to assess chronic stress, worth understanding honestly rather than trusting a single number at face value.

The HPA axis, and how cortisol is actually assessed

Cortisol is the output of a three-part feedback loop called the hypothalamic-pituitary-adrenal axis. Understanding the loop explains both why a single measurement is close to meaningless and what a proper assessment looks like.

How the loop works

The hypothalamus releases CRH, which prompts the pituitary to release ACTH, which prompts the adrenal cortex to release cortisol. Cortisol then feeds back to suppress the first two steps. It is a self-limiting system, which is precisely why the popular idea of adrenal glands becoming exhausted from overuse doesn’t match the physiology.

The daily curve

Consumer stress tests sampling a single point therefore have limited diagnostic value, whatever the marketing says.

Cortisol follows a strong circadian rhythm. It rises sharply in the 30 to 45 minutes after waking — the cortisol awakening response — peaks in the early morning, then declines across the day to a trough around midnight. The size of that morning rise, and the steepness of the daytime decline, carry more information than any single value.

A flattened curve, where the morning rise is blunted and the evening value stays elevated, is the pattern most consistently associated with chronic stress in the research. You can’t detect a flattened curve from one blood draw, which is the entire problem with how cortisol is usually sold.

The four testing methods

  • Serum cortisol. A blood draw, usually morning. Measures total cortisol including the protein-bound fraction. Standard in clinical investigation of Cushing's or Addison's, poor for assessing everyday stress.
  • Salivary cortisol. Measures the free, biologically active fraction. Non-invasive enough to sample repeatedly, which is what makes curve assessment possible. Typically four to six samples, following the sampling protocol set out by Stalder and colleagues, across a day. Proper characterisation needs multiple measurements across multiple days rather than a single sampling session.
  • 24-hour urinary free cortisol. Integrates total output across a full day, losing all information about timing.
  • Hair cortisol. Reflects cumulative exposure over months. Genuinely interesting for chronic stress research and still largely a research tool.

Why adrenal fatigue isn’t a recognised diagnosis

The Endocrine Society has stated explicitly that adrenal fatigue isn’t a recognized condition, and Cadegiani and Kater’s systematic review found no consistent evidence that the proposed cortisol pattern exists in people given the label. Genuine adrenal insufficiency, Addison's disease, is a real and serious diagnosis with clear biochemical criteria — and it is rare.

The symptoms people describe are real. The proposed mechanism isn’t supported, and pursuing it usually delays investigation of the more likely explanations: inadequate sleep, thyroid dysfunction, anemia, depression, or simply a training load exceeding recovery capacity.

What to track instead of cortisol

The usual framing sells single-point cortisol tests as a definitive stress score. Cortisol follows a strong daily rhythm, so one measurement captures a point on a curve rather than your state.

Resting heart rate and sleep are more reliable downstream signals, and both are far easier to track repeatedly.

If cortisol is genuinely in question, the pattern-based testing a clinician can arrange tells you something. A one-off consumer test doesn’t.
