The everyday part is a mostly plant-based, pescatarian pattern: vegan with the addition of fish two or three times a week, complex carbohydrates from vegetables, whole grains and legumes, and unsaturated fats predominating, with olive oil featuring heavily. Protein is set deliberately low at roughly 0.7 to 0.8g per kilogram of ideal bodyweight, most of it from plants.

The periodic part is the fasting-mimicking diet, a structured five-day protocol run three or four times a year rather than continuously. Longo's framing is that this delivers the cellular responses of fasting without the sustained deprivation.

Behind both sits his “Five Pillars of Longevity”, a framework that weighs evidence from several research areas rather than any one, which is the part of the book that ages best.

DASH is worth noting for what it illustrates. It was never designed as a longevity diet, it was designed to lower blood pressure, and it works. Since blood pressure is one of the strongest modifiable risk factors there is, a diet that moves it is doing longevity work under another name.

What the longevity diets all share

Set the branding aside and the same six features appear in every pattern with real evidence behind it.

  • Mostly plants. Vegetables and legumes as the base rather than the side.
  • Legumes as a staple. The single most consistent food across all of them.
  • Unsaturated fat as the main fat. Usually olive oil, sometimes nuts.
  • Fish more often than red meat. Not necessarily no red meat, just less of it.
  • Very little ultra-processed food. This may be doing more work than any positive inclusion.
  • An energy intake that keeps central fat low. Where fat sits matters more than what the scale says.

That list is unglamorous, which is part of why the named diets exist. A pattern with a name and a book is easier to sell than six bullet points, and easier to follow.

Where they genuinely disagree

Protein is the real fault line, and it is worth being straightforward that this site takes a different position from Longo.

His protocol sets protein at roughly 0.7 to 0.8g per kilogram of ideal bodyweight. The protein evidence covered here supports 1.2 to 1.6g/kg, with older adults at the higher end rather than the lower, per the ISSN Position Stand and the PROT-AGE Study Group. That is close to a twofold difference on the same question.

The disagreement is real rather than a misreading. Longo's case runs through growth signalling: lower protein means less IGF-1 and mTOR activity, and in animal models that pathway tracks with longer life. The opposing case runs through sarcopenia: aging muscle responds less efficiently to the same dose of protein, so older adults need more to maintain what they have, not less.

Both mechanisms are well documented. They point in opposite directions for the same person, and the trials that would settle it in humans have not been run.

My own read is that the muscle case is the safer bet for most people over 40, because losing muscle has an immediate and measurable cost to function while the growth signalling benefit is inferred from animal work. Longo would disagree, and he has spent longer on the question than I have.

The criticisms

Three come up often enough to address.

The protein target. Covered above. For an older adult already at risk of muscle loss, a low-protein prescription is the part of the protocol most worth questioning with a doctor or dietitian.

The weight carried by animal research. Much of the mechanistic argument rests on work in yeast, mice and other models. That work is real and it is not the same as a human outcome trial.

The commercial link. The fasting-mimicking diet is sold as a branded five-day product by a company Longo founded. He has stated that his share of the proceeds goes to a foundation, and readers can weigh that as they see fit. Knowing the arrangement exists is more useful than not knowing.

None of these makes the underlying eating pattern a bad one. Stripped of the protocol, it looks a great deal like the Mediterranean pattern, which has the better evidence anyway.

What to take from all this

Take the shared six features and stop there. They cover almost everything the named diets agree on, and they require no book.

If having a named plan helps you stick to something, pick the Mediterranean pattern. It has the best evidence, the widest range of foods, and the fewest rules to break.

Adherence is the variable that matters most. A pattern you follow for a decade beats a better pattern you abandon in March, and most of the difference between diets in the research is smaller than the difference between following one and not.
